Comment 14 for bug 808865

Dear Carl,

We would need to create two new fields in the Examination form to
facilitate the use of the Examination Card template:

1. Examination Title - mapped text field.

2. Examination Centre - mapped text field.

Can this be added to the list of to do in a separate bug report or can
it be tracked in this bug report?
