The most important thing is to know if this is reproducible with the latest Simple Scan from Ubuntu 12.04 and if it is reproducible every time or just occassionally.

This seems to be a Hardware Issue, i.e. Simple Scan does not support your scanner perfectly -- or possibly not at all.
