This is a combination of two known issues we want to work on in the next release:

Smarter dealing with attachments is something we want to attack in 0.3.

As for choosing to download attachments, I can see that, especially if Geary asked only if the attachment(s) were of a large size. I'm not sure it makes sense for Geary to ask about every little attachment. I've ticketed that here: http://redmine.yorba.org/issues/6086
