The web form currently has a "Nickname" field. This should be "Name on badge". It can continue to be auto-filled from given & family name.

The "Badge line 1" and "Badge line 2" fields are also necessary. We cannot assume that any particular piece of info should be on a badge. In other words, they should be independent fields. Some people will want no more than their name. Others will put their email address and/or company and/or project etc. Some will NOT want one or more of these bits of info on their badge. We should allow this flexibility.

Please group these 3 fields together, and order them as on the badge: name, line 1, line 2.
