if you can, and you don't need the LTS version, try using Ubuntu server 6.10. I've made all my tests using that release. I think Ubuntu people committed some bugfixes on SILO since release 6.06.1. The other thing I can suggest is to be sure to intercept the Initalizing Memory process as soon as possible in the boot process.
I wasn't able to understand the problem in deep. It is only a "it works for me" solution :) but I've repeated the procedure many times on Ultra 60 machines with always the same result.
