We've had plenty of successful runs on this kernel. I suspect io load or something else may trigger this. We've only seen it once out of tens of not hundreds of runs on Cosmic 4.15.0-29-generic.
We don't currently have an easy way to test upstream kernels in the automation runs. If we see this again, I'll poke a bit more on running mkfs.ext4 over bcache devices on Cosmic to see if we can recreate outside of the automation.
