The display driver was not providing enough contrast to allow the menu selection to be visible.
Installed the ATI closed-source driver - see the washed out display question.
Rebooted. The system came up normally.

Everything seems to work.
The problem was replacing the ATI driver with the open source driver and making the default window manager twm.
