hdrs in use refers to how many nsMsgHdr objects are in memory for that folder. It helps us know how much more memory is being used for that folder, and also what the folder is being used for (things like views and gloda indexing create nsMsgHdrs when they use a db; get/setting a property on a folder in the db doesn't cause any headers to get instantiated)
