Have the same issue on 16.04 (kernel 4.4.0-66) with a Realtek ALC892 card. acpi_listen shows multiple headphone plug/unplug events:
jack/headphone HEADPHONE plug jack/headphone HEADPHONE unplug
The volume dialog popping up is a consequence of the system thinking a jack was plugged in. Moreover, I find that once I plug in headphones, the spurious jack events stop. Is this a Pulseaudio bug, or something in ACPI/further upstream?
