I'm having the same issue on precise 12.04 (upgraded from lucid):
# virsh snapshot-create-as lucid-server backup --disk-only error: internal error unable to execute QEMU command 'blockdev-snapshot-sync': An undefined error has ocurred
libvirt log shows the same:

Same as Uri the virt machine has a qcow2 disk that I can snapshot ok manually using qemu-img.
