As mentioned in the duplicate, the elementary-icon-theme package itself also ships:

 /usr/share/icons/elementary/status/48/untitled folder/

Upstream rejected the fix of just deleting the folder (in bug #786978) as it might cause issues with the panel icons. Will need to do some testing...
