In fact this bug is reproduced only if we have more than one evacuation attempt so there are more than one row can be found by filter instance_
Nova manager marks migration as 'completed' at the first start after failure but because of error in code nova manager finds only last row so therefore instance will be rebooted at nova-compute restart several times which equals the number of 'accepted' evacuation attempts.
