Modify group members fails when htdocs used as document root

Bug Description

Distrubution: Ubuntu 8.04.4 LTS
Package version: phpldapadmin 1.1.0.4-2ubuntu1

Expected behaviour:

The "modify group members" form (modify_member_form.php) should be a well-formed XHTML document with a single <head> section, which properly includes the javascript required to drive the form.

Actual behaviour:

When htdocs is used as the Apache (apache2 2.2.8-1ubuntu0.17) DocumentRoot, a duplicate doctype, <html>, <head> etc. are served, producing an invalid XHTML page, the javascript libraries are not included, and the page fails to operate correctly.

When /usr/share/phpldapadmin/ is used as the DocumentRoot, the page renders and functions correctly.
