Layout of GTK Debconf prompt is ugly

Bug Description
debconf 1.5.40ubuntu1, Ubuntu 11.10
The GTK interface for Debconf prompts is unattractive. In particular:
* the title of the window is "Debconf on {computer hostname}", which is not meaningful
* it has a bright orange banner across the top
* the banner forms the top of a 1px orange border that touches the edges of other elements in the window
* there is no padding between the banner and the text immediately below it
* it has a "Forward" button, when there's nothing to go forward to (or back from).
This bug will be fixed when:
* the title of the window is human-readable
* there is no bright orange banner
* either there is no border around the window contents, or there is proper padding between the border and all other elements
* there is proper padding between the title text and items below it
* the "Forward" button is relabelled to "Continue".
