This icon is not part of the xdg icon scheme. The programs in question are at fault in this case, as they don't depend on either gnome-icon-theme or humanity-icon-theme. Alternatively they could use the standard "unknown" icon that all xdg-compliant icon themes have.

affects: oxygen-icons (Ubuntu) → ubuntu

wouldn't that mean, the user needs to use another theme to use the program?

Thank you for taking the time to report this bug and helping to make Ubuntu better. Unfortunately, we cannot work on this bug because your description didn't include enough information. You may find it helpful to read "How to report bugs effectively" http://www.chiark.greenend.org.uk/~sgtatham/bugs.html. We'd be grateful if you would then provide a more complete description of the problem.

We have instructions on debugging some types of problems at http://wiki.ubuntu.com/DebuggingProcedures

At a minimum, we need:
1. the specific steps or actions you took that caused you to encounter the problem,
2. the behavior you expected, and
3. the behavior you actually encountered (in as much detail as possible).
Thanks!

I have installed blueman on kubuntu. When trying to start it I got the gtk-missing-image is missing error report.

Solution is to install gnome-icon-theme. After that blueman started as expected. I believe this means that blueman should depend on gnome-icon-theme.

affects: ubuntu → blueman (Ubuntu)

blueman packages with itself a bunch of icons, i'm sure it's much better to pack all dependent icons rather than link dependency to third party packet which is not obliged to carry required hardcoded images.

Confirm this bug on Kubuntu 13.04 - can't start blueman-applet.
sudo apt-get install gnome-icon-theme
don't solve the problem, I see the error:
Failed to load StandardItems
Icon 'gtk-missing-image' not present in theme

Did anybody know some solution for this issue?

Problem solved for me after going to System Settings - Application Appearance - GTK and changing Icon Themes - Fallback theme to GNOME (default was be Oxygen).
