polygon plows should trim areas connected < min width

When polygon plowing creates slivers smaller than the minimum trace size, the sliver should be removed. If that creates an island, the isolated part of the polygon should be removed (as normal). Otherwise the DRC/ratsnest may rely on even microscopic slivers to consider a polygon connected.
