Bug Description
When adding a new archive subscription for a private PPA, the page at archive/
The form should be hidden via JS, and only be displayed when the person clicks on 'New subscription'
Also, beuno mentioned it might be better if the form was not part of the table, so move out to be a standard LPForm.
