codeofconduct page does not point to which version we have signed

Bug Description

On the codeofcondut page ( https://launchpad.net/codeofconduct ) we have the last version on the top of the page and just below the sentence "Congratulations, you have already signed the Ubuntu Code of Conduct.". this let us think we have signed the last version, but it's not true for lot of launchpad users.

It sould be more helpful to have a sentence inviting users to sign the new code of conduct if they have only signed an old one and to point the version(s) already signed (on the list of all the versions for example).
