copyright notices not up to Debian policy

Bug Description

Tim informs us that the debian/copyright files we have would not be accepted by Debian.

7) your debian/copyright notices would be REJECTED by Debian for not
naming the individual copyright holders, not naming the individual
authors, and not including an upstream URL. It may not be worth your time
to maintain such information at this time, but I figured I'd mention it in
case you were not aware.

We should fix this sometime. With few exceptions they should all just come from a single file in the SVN repo.

Also, our individual source files should probably have a copyright+license notice. A handful of files attribute copyright to "the MIT Student Information Processing Board", which isn't really right, and the rest have nothing.
