Redo display of health worker records

When the record is first opened, display only the most important information (name, position, facility or contact info, photo). Then display tabs or icons linking to different parts of the health record that open when clicked, while the name info remains constant. The purpose is to avoid scrolling and make records easier to navigate. This is primarily a UI change.

The record will still need to be printable.
