This was because I pushed the "Post your comment" button twice by mistake. Prevent this from happening.
Maybe giving each comment a unique id (which is generated when code if that comment textbox is generated) and checking that id on the server side, is a solution.

Do we want to stop the user ever making an identical comment on an item or just not let them make a comment identical to the last comment if they were the last commenters.
I can imagine that we should allow the following conversation..
John Smith: I think that xxxxxxx
Jane Jones: +1!
Peter Griffin +1 I think that xxxxx and yyyyy too
Jane Jones +1!
