Annotation title uniqueness - is it still needed?

Bug Description
In the current system, annotation documents are required to be unique. I believe the reasoning was that, with the current design, if you wanted to have the same tag apply to two different spans, you could simply assign the same annotating document to two different spans of text, and it would be done. This forces taggers to keep all annotations of the same type limited to one record, which makes it easy to then do changes to the annotation content. But I don't think we currently support adding additional spans to an existing annotation, nor do we allow multiple spans to be selected when an annotation is created, so in fact, neither system works.
We need to make a decision: do we force single, multi-targetted annotations? Or do we allow for duplicate annotations? Either way, some UI will need to be built to support the decision.
I can work with either system, so I'm afraid I don't really have an opinion on this one.
