Bug Description

Strangely, I cannot see the "Open..." icon in the toolbar of LibreOffice Writer (ver. 4.1) although I can see it in Calc and Impress.

Getting stranger...
Please find attached my screenshot with Calc and Writer windows.
I made further tests, and saw that:
1) None of standard LO icon styles has this problem.
2) None of other, 3rd party LO icon styles (Faenza, FaenzaDark, Flat, SolusOS) has this problem either.
