Ubuntu login support - takes me to Canonical Global Services Support?

Bug Description

When I went to request support for Ubuntu login I was taken to a support page with several options in a dropdown box. I selected what I thought was the best option and it gave me a FAQ entry. Once I clicked "submit" I was never given an option to enter any data and was presented with a "someone will be in touch with you from Canonical Global Services Support.

I *should* be given an option to enter data and be sent to Ubuntu login support (not sure what global support is...).

This is an old, old issue... the support forms were created before SSO had any support outside of the commercial paid support team. It seems like a good candidate for inclusion in our current rebranding effort, to improve user experience and prevent tickets from going to people who don't really want to include SSO in their support scope.
