Bug Description
I accidently uploaded a pdf when we already had a pdf in the system (via the 'file' option in the submit form rather than via the 'pdf' option), which really highlights that we probably need to re-think the submit form in future and also possibly add an option to remove a submitted file (as long as the document isn't 'available').
