I see labels like so:
Full name:
Ubuntu One email address:

(Optional)
A 256x256 image for your profile.
Package namespace::
(Optional)
PayPal email::
(Optional)
If different from your Ubuntu Single Signon email.
Phone number::
(Optional)
Street address::
(Optional)
City::
(Optional)
State/Province/
(Optional)
Postal code/Zip::
(Optional)
Country::
Company name::
i.e. Full name and E-mail address have a single colon, all the other fields have two colons.
