When removing the application, Keystone endpoints are not removed

Bug Description
When I remove the Cinder application from an Openstack cloud, I expected the remove hooks to go about clearing the endpoints from the Keystone catalog - however that's not the case and we have to manually remove the endpoints. Although this may be by design (the volumes, database, etc are also not removed), it might be helpful for operators to have the (easily replaceable) endpoints removed.
