[doc] User Reference manual lacks an index

Bug Description

The Bazaar User Reference manual does not have an index, at least not in the Windows .chm files installed by the standalone installer.

Without an index, searching for a topic you are interested in is almost impossible.

Please add an index.

Yes, there is search function, but it will only succeed if the phrase you type occurs verbatim in the text. Even a slight variation of wording will cause a complete failure. IOW, you are at the mercy of the authors.

OTOH, an index is made with search in mind, and it normally includes common phrases people tend to think about when they are looking for some subject.
