bzr upgrade --rich-root-pack fails badly when upgrading subtree repositories

Bug Description
Had a user today who ran 'bzr upgrade --rich-root-pack', which blew up
leaving:
A good backup in .bzr.backup
No .bzr/repository directory
This confused them, because the upgrade command did not report an error
either, only that the format was already meta-dir-1.
We should probably check for compatibility, or handle compatibility
errors with a rollback.
