Add "User manual" menu item to Help menu

Bug Description

Many users are not aware that we deliver a manual with every download of Mixxx. It should be more easily to find and open that manual. The new Menu item should simply open the Mixx manual pdf.

We don't include the manual in the Mixxx bundle on OSX.

To make matters worse, we store the manual in a different place (relative to the config directory) on Windows and Linux.

OK, I special-cased per OS. If the manual file isn't present then it just opens a web browser to http://mixxx.org/manual/1.10 (we have to remember to update this URL in defs_urls.h for every release.)

Can we put a macro in defs_urls.h to yank the major version number from defs_version.h so we don't have to update both?
