Is still present in the Precise12.04-LTS squid3, 3.1.15-1ubuntu2, which has become the main version of squid as of Precise12.04.
This is a one-line-fix (though I had to merge it manually):-

It would be good to have the fix included in the Precise12.04 release, for those who rely on miss_access rules.

This has been patched in a Debian upstream version of Squid3. Resync'ing fixes the problem.
