Bug Description
Binary package hint: appmenu-gtk
This is similar to (and may share implementation details with) bug 696896, but I'm filing a separate bug because the two kinds of items are treated differently (toplevel vs nested) and may be fixed separately.
Any toplevel menu item that dynamically swaps out its submenu will correctly have its submenu removed, but the new menu will not appear. You just get an empty menu with no items.
You can see this in Empathy's chat windows: the Contact entry is blank.
And Eye of GNOME when you toggle the "Reload Image" plugin, the Tools menu is created, but is blank. Restarting the app will create it correctly.
