Allowed sreg checkboxes in rpconfig admin interface do not display correctly

The 'Allowed sreg' checkboxes in the admin interface of the rpconfig page do not display correctly (at least in Firefox 3.6.3 and Chromium 5.0.375.70)

What I see:

The checkboxes are misaligned and all checkboxes are unchecked. See attached screenshot.

What I should see:

The checkboxes should be aligned and the fields that are selected should be checked and the fields which are unselected should be unchecked.

How to reproduce:

 1. Login to the rpconfig admin interface
 2. Select a test service
 3. Scroll down to the 'Allowed sreg' field
 4. See how the checkboxes are misaligned
 5. Check some (or all) of the checkboxes
 6. Click the 'Save' button.
 7. Select the same test service
 8. Scroll down to the 'Allowed sreg' field
 9. Notice that the checkboxes are all unchecked

Note:

I have verified in the database which fields should be selected (checked) to make sure. By checking (or leaving unchecked) checkboxes and clicking 'Save', the values are correctly saved in the database. This seems to be a display problem only, but impacts usability rather heavily.
