Bug Description

As outlined by Dan Reverri (https://answers.launchpad.net/txamqp/+question/74864), txAMQP should clear the channels dictionary when closed.

I've added Dan's patch, but I'm not sure if a race condition may occur. Should connectionLost acquire channelsLock?
