Multiple-options campaign message boxes

Bug Description

It would be nice to be able to show message boxes in scenarios that allow the player to choose between several options.
There could be various types, such as messages with…
 – checkboxes, where the player can select any combination of entries
 – a dropdown menu or radio buttons or several buttons, so the player has to select exactly one option
 – a text field for free entering of text
 – a spinner to select a number within a range

For the text dialogue: I´d like to ask the player his name once when he starts the first mission and then address him by name throughout the campaign, to integrate him into the storyline like one of the characters. For example, if you then play the frisian campaign, the chieftain will be called not Reebaud but kaputtnik ;)
I also want to give players the chance to choose between several difficulties (like easy/medium/hard) when they begin a campaign. Spinners could be used to allow players to set advanced settings for the scenario difficulty if they like, such as the number of wares demanded in fri02.

I think the difficulty could also be selected in on the campaign or campaign scenario selection screens rather than in-game.

I'll put up https://code.launchpad.net/~widelands-dev/widelands/campaignselect_box for code review first when we have build 20 ready so that we can continue work there.
