Bug Description

Due to java script from the previous page is not loaded to the page, opened in new window - nested application creation is not possible.

Need to catch this, prevent creation of such forms and show notification about that.

    Make add-select work on static pages

    Before it was impossible to 'Add application' button from statically
    rendered form page. This happened, because js callbacks were only
    executed during modal form init. This patch adds an additional call of
    js code in add-select in case the page being rendered is a static one.
