Users have to enter login information several times before login accepted

Bug Description
Starting yesterday some users started to complain that they have to enter their KARL login several times in a row before they can login. Eventually it would work however. The username and password for these users is correct. It's very strange. I initially assumed the users were just typing their login incorrectly, but that's not the case because I started experiencing this as well. You enter the login and click submit, then screen refreshes and nothing happens. You enter it again - nothing happens. Then finally on the 3rd try it works (or sometime it took me up to 5 tries). I can get this to happen across browsers, but I can't get it to happen consistently, so I don't have a repeatable set of steps that will trigger the error. There is definitely a problem though because I have complaints from 22 users in several different offices (including National Foundations who sit outside the core office firewall), so I think this is something on the KARL side.
Do you think this could have something to do with the "came_from" change?
