Filled-out fields are blank when printing PDF form

Bug Description
If a PDF form is filled out using acroread and saved, then printing the resulting pdf using 'lpr'
results in a blank form, with none of the filled-out field data.
However the filled-out field data -does- show if the file is first opened with evince and printed from within evince. Ditto for acroread.
Attached is a sample form. It has been filled out with "blah blah" in the first few fields
(view it with evince or acroread to see for yourself).
