Support 'bzr commit --fixes lp:xxx' in the actual commit message

Bug Description
I'm aware of the bzr commit --fixes option.
However, I would find it far more convenient to place a well formed string inside of the commit message itself, like "Fixes: #123456", or "LP: #123456".
I generally forget about the --fixes option until I'm the editor writing up the changelog entry. Also, I find that if I do use the --fixes option, I end up duplicating the information in the changelog entry.
