a restricted title mode to prevent duplicates

There should be an option, or it should be the default actually, that titles with namespaces can't be duplicate. This would prevent an error with somebody creating the same item.

For non-namespaced items it could be optional, or simply not supported.

Note that multi-language will be supported at some point, and those might have the same title.
